By submitting this form, I do irrevocably grant the Colorado Department of Public Health and Environment (CDPHE) and its vendors / project associate SE2, the right (yet not obligation) to utilize my appearance, any still photographs or video footage of me, the recording of my voice, any writing or other content that I have submitted to SE2, and my name in the creation of website content, social media and campaign advertisements. I understand that the final products may be broadcast on CDPHE’s digital properties (Facebook, Instagram, X, etc.) or published in magazines, newspapers or other print publications in an effort to support tobacco users to consider a quit attempt / utilize the Colorado Quitline for cessation support. I also understand that CDPHE will have full ownership of the finalized deliverable along with any unused raw footage / photos / audio upon the completion of this project. Audio, video, film and/or print images that I provide may be edited, duplicated, distributed, reproduced, broadcast, and/or reformatted any form and manner indefinitely. I hereby release, discharge, and agree to hold harmless SE2 on behalf of CDPHE, and their legal representatives or assigns, and all persons acting under their permission or authority, or those for whom they are acting, from any liability I connection with the use of the images as aforesaid or by virtue of any alteration, processing or use thereof in composite form, whether intentional or otherwise, as well as any publication thereof.
